Sharing information

Key Information Summary (KIS) has been designed to support patients who have complex care needs or long term conditions.

KIS allows inportant information to be shared with health care professionals in uncheduled care.  Information is shared with patient consent and KIS will be available for users within NHS24, out of hours organisations, A & E departments plus hospital and pharmacy environments.

KIS contains special information from the GP practice and also:

  1. Future care plans
  2. Medications and allergies
  3. Current situation, including main diagnosis and current issues
  4. Carer and support details including next of kin
  5. Information and recommended action for out of hours clinicians
  6. Your wishes

In the future, KIS will also be used in scheduled care for patients with long-term conditions; for example, for those who regularly visit renal clinics and within the Scottish Ambulance Service.
